Referral criteria

Referral types, intake requirements, and IAR-DST alignment for each service at Primed Health.

Referral criteria are reviewed regularly. For the most current information or to discuss a specific patient, contact our intake team on 1300 404 010 or [email protected].

Criteria by service

Medical Specialist

Expert specialist care for complex medical needs

IAR-DST Level 3–4

Accepted referral types

  • GP referral (required for Medicare rebate)
  • Specialist-to-specialist referral

What to include in your referral

  • GP referral letter (valid for 12 months for specialists, 3 months for GPs)
  • Relevant investigation results (pathology, imaging)
  • Current medications and allergy list
  • Relevant specialist letters and clinical history

A valid GP or specialist referral is required for Medicare rebates on specialist consultations. Referrals should be addressed to Dr Kerryn Loftus or "Primed Health Medical Specialist".

Psychology

Evidence-based psychological support for mental health and wellbeing

IAR-DST Level 2–3

Accepted referral types

  • GP Mental Health Treatment Plan (MHTP)
  • Specialist referral
  • Self-referral (no Medicare rebate)

What to include in your referral

  • Mental Health Treatment Plan from GP for Medicare rebates (up to 10 sessions per calendar year)
  • Referral letter stating presenting concerns and relevant history
  • Current medications and treating team details
  • Any previous psychological assessments or reports

Patients may self-refer without a GP referral, however Medicare rebates require a valid MHTP. We accept referrals for adults (18+).

Allied Health

Multidisciplinary allied health for whole-person care

Accepted referral types

  • GP referral
  • Specialist referral
  • Self-referral
  • NDIS plan (where applicable)

What to include in your referral

  • Referral letter with presenting concerns and relevant clinical history
  • Current medications and treating team
  • Relevant imaging or investigation results (where applicable)
  • NDIS plan details if NDIS-funded

Allied health services do not require a referral for private billing. A GP or specialist referral is required for Medicare-subsidised services (e.g. Chronic Disease Management plans).

How to send a referral

HealthLink SmartForms

Send a structured electronic referral via HealthLink. Search for "Primed Health" in your practice management system.

Fax or letter

Post or fax a referral letter to our Frenchs Forest clinic. Include all relevant clinical history and investigations.

Phone our intake team

Call 1300 404 010 to discuss a patient before referring. Our intake team can advise on the most appropriate service.
